Life & Disability Benefits

EHS continues to offer additional benefits to you automatically, at no cost, depending on your eligibility. In some instances, you have the opportunity to supplement this automatic coverage or choose a higher level of coverage, if you’d like.

Short Term Disability:

EHS automatically provides full-time employees, at no cost, 60% of their weekly base pay ($2,300 maximum weekly benefit) after a 45-day waiting period. You can elect a shorter period – either 15 or 30 days. You can increase your coverage by one benefit level only each plan year.

Long Term Disability:

EHS automatically provides full-time employees, at no cost, 60% of their annual base pay ($10,000 maximum monthly benefit), or you can elect from the supplemental option of 70% of your annual base pay ($10,000 maximum monthly benefit).

Life & Accidental Death & Dismemberment Insurance:

EHS automatically provides full-time employees, at no cost, basic life coverage of 1 times the annual base salary to a maximum of $500,000 ($20,000 minimum) plus AD&D. Part-time employees may choose to purchase life and AD&D at cost.

Full-time employees may elect 1, 2, 3, 4, or 5 times your annual base pay ($1 million maximum benefit) as supplemental coverage. You can increase your insurance coverage by one benefit level only each plan year.

Note: You must provide Evidence of Insurability (EOI) and receive approval before coverage goes into effect if:

  • Your election provides a coverage amount in excess of $500,000;
  • You newly elect 4 or 5 times your base pay; or
  • You are over the age of 60 and elect Supplemental Life Insurance for the first time

Dependent Life Insurance:

EHS employees can elect supplemental life insurance for their dependents.

For your spouse: $10,000, $25,000 or $50,000. EOI is required for an election of $50,000. Part-time employees are eligible for $10,000 only.

For your child(ren): $5,000 or $10,000
