Carefully review each of the sections below and follow all instructions appropriate to you.
A) Please take a few moments now to log in to your onboarding portal and complete the assigned documents.
B) Notes about completing your onboarding documents:
If you are interrupted while completing a document, simply save your work prior to exiting. You will be able to continue where you left off the next time you open that form.
You must click the Send to HR button at the end of each form when you have finished.
C) In addition to completing the forms found in your onboarding portal, please carefully review and print the following informational documents:
__ Employee Health Medical History Form (you must bring this completed form with you to your appointment with Employee Health)
__ Release of Information Form (you must bring this completed form with you to your appointment with Employee Health)
__ OSHA Mandatory Respirator Questionnaire and TB Testing Consent (you must bring this completed form with you to your appointment with Employee Health)
D) If you have been hired as an RN, LPN, LNA, or Medical Assistant, please print and refer to the following additional orientation information:
__ Hospital Staff Orientation Schedule (RN and LNA)
__ Outpatient and Specialty Practice Orientation Schedule (RN, LPN, and MA)
__ Outpatient and Specialty Practice Medication Exam Study Guide (RN, LPN, and MA)
__ VNA Staff: Orientation schedule will be emailed to you prior to start date
